Submission Forms

Prior to sending us tissues fixed in formalin (via “Courier pick-up” service, or “Mail”), please fill out “Submission form” for each patient.
Ideally, the filled “Submission form” should be printed and sent to us along the tissue specimens.
Alternatively, the “Submission form” can be e-mailed to us: tlvetpath@gmail.com
